Functional Medicine Heartburn Care
The 3-Step Root Cause Heartburn Protocol — find the cause, heal the gut, and rethink the medication.
Functional medicine heartburn care addresses the underlying drivers of reflux — H. pylori, food sensitivities, low stomach acid, stress physiology, and gut microbiome imbalance — instead of suppressing acid indefinitely. In our Wilmington practice, we run a structured 3-Step Root Cause Heartburn Protocol: a free discovery call, targeted advanced testing, and a personalized plan built from your results.
Why "Just Take the Pill" Keeps Failing You
You have probably heard it before. Take the antacid. Take the PPI. Avoid spicy food. Come back if it gets worse. For some people, that works. For the people who find us, it did not.
Here is the problem: acid suppression quiets the symptom without answering the question. Why is acid irritating your esophagus in the first place? Sometimes the answer is too much acid. Often it is not. We regularly see reflux connected to an H. pylori infection, a food the immune system has started reacting to, weak digestion from low stomach acid, cortisol patterns that have disrupted gut motility, or bacterial overgrowth in the small intestine. Suppressing acid does not fix any of those.
The good news: once we know which one is driving your case, the plan gets much more specific — and much more doable.
The 3-Step Root Cause Heartburn Protocol
Step 1: Discovery Call (Free, 15 Minutes)
Every case starts with a conversation. On your free 15-minute discovery call, we listen to your history — when the heartburn started, what makes it better or worse, what you have already tried, and what testing you have had. We tell you honestly whether we think we can help and what testing would look like for your case. No cost, no commitment.
Step 2: Advanced Testing
Step two is where we stop guessing. Depending on your history, we use some combination of:
- Comprehensive Stool Analysis — gut microbiome testing for bacterial overgrowth, parasites, H. pylori, and digestive markers such as elastase and inflammation. Details on our stool testing page.
- Alletess 184 Food Sensitivity Panel — immune (IgG/IgE) responses to 184 common foods, to surface hidden sensitivities stoking inflammation. See food sensitivity testing.
- Adrenocortex Stress Profile / HU-Map — cortisol rhythms and stress hormones that shape acid production, gut motility, and the lower esophageal sphincter. See adrenal stress testing.
- H. pylori testing — stool antigen and functional panels. We look carefully here because what we see clinically is that conventional breath tests can miss strains when biofilm is present.
Most kits are collected at home and mailed to the lab. Results are typically back within 7 business days, and then we schedule a dedicated results review — a full appointment just for walking through what your labs say.
Step 3: Your Personalized Plan
Your plan has three parts: eliminate triggers, heal the gut, and reduce medication with supervision. We remove the foods or infections your testing identified. We support the gut lining and digestion with what your markers show you are missing. And where long-term acid suppression is part of your story, we build a gradual step-down plan with your prescribing doctor — never on your own, always coordinated. For more on that piece, see PPI cessation support.

- H. pylori — a common stomach infection that disrupts acid regulation and irritates the stomach lining.
- Food sensitivities — often hidden; the immune reaction to a food can show up as reflux hours later.
- Low stomach acid (hypochlorhydria) — weak digestion that leads to fermentation, pressure, and reflux.
- Adrenal stress dysregulation — cortisol rhythms that alter motility, acid output, and sphincter tone.
- Eosinophilic esophagitis (EoE) — an immune-driven inflammation of the esophagus that mimics stubborn GERD.
- Gut microbiome imbalance — SIBO and dysbiosis that generate gas and pressure after meals.
- Hiatal hernia — a mechanical factor we account for rather than pretend diet alone will fix.
Conventional Care vs. Our Approach
| Topic | Conventional approach | Our functional approach |
|---|---|---|
| First move | Acid suppression medication | Testing to identify the driver |
| Testing scope | Scope and structural imaging | Stool, food sensitivity, cortisol, H. pylori panels |
| Question asked | Is there damage to treat? | Why is the reflux happening? |
| Food role | Avoid triggers list | Identify your specific reactions by immune testing |
| Medication | Often open-ended | Gradual supervised step-down when appropriate |
| Follow-up | As needed | Dedicated results review and ongoing plan |
Who This Protocol Is For
- Chronic heartburn or GERD that persists on medication
- Reflux that flares with stress or after specific meals
- Bloating, gas, constipation, or diarrhea alongside heartburn
- A normal endoscopy with no real explanation
- Anyone tired of managing symptoms without knowing the cause
We see patients in Wilmington and across North Carolina via telehealth. Our practice, Wilmington Functional Medicine, has helped hundreds of patients over more than 20 years of practice, and this protocol is the distillation of that work. What is the 3-Step Root Cause Heartburn Protocol?
It is our structured process for chronic reflux. Step one is a free 15-minute discovery call. Step two is targeted advanced testing — stool analysis, food sensitivity panels, cortisol testing, and H. pylori testing as your case requires. Step three is a personalized plan to eliminate triggers, heal the gut, and reduce medication through a supervised step-down with your prescribing doctor.
How long does functional medicine heartburn care take?
Testing results are typically back within 7 business days, and your results review follows shortly after. From there, most patients work through their plan over a few months, with check-ins along the way. Timelines vary with how long you have had symptoms and what the testing uncovers. We set expectations clearly at the start.
Will I have to give up my PPI?
That is a decision for you and your prescribing doctor, not us. What we offer is testing that shows why the reflux began and a gradual step-down plan we coordinate with your doctor if reducing medication is appropriate. Many patients want off long-term acid suppression, and doing it safely requires support and data.
Is functional medicine heartburn care covered by insurance?
Lab testing may be eligible for insurance or HSA/FSA reimbursement depending on your plan, and we give you the details up front. The discovery call is free either way. We would rather you understand the costs before committing to anything — ask us on the call and we will walk through it plainly.
What if testing does not find anything?
It happens occasionally, and we tell you what it means rather than stretching the results. Sometimes it points us toward a mechanical factor like a hiatal hernia, and we say so and refer appropriately. You will never get a plan built on findings we do not have.
