Comprehensive Stool Analysis & Gut Microbiome Testing
A single sample tells us what is living in your gut, how well you are digesting, and what is fueling your reflux.
A comprehensive stool analysis measures the gut microbiome and digestive function — bacterial overgrowth, H. pylori, parasites, digestive enzymes like elastase, and inflammation markers — to show what is driving your heartburn. In our Wilmington practice, we use it to connect reflux symptoms to specific gut imbalances, so your plan targets the actual problem instead of another guess.
What This Test Actually Measures
Stool testing today is nothing like the simple "culture and see" test of decades past. The panels we use read your gut ecosystem in detail. Here is what comes back:
- Communal gut microbiome analysis — which bacterial families are dominant, which are depleted, and whether the balance favors fermentation and gas production after meals.
- Bacterial overgrowth markers — evidence of SIBO-type patterns and dysbiosis that create pressure pushing stomach contents upward.
- H. pylori detection — stool antigen testing for the stomach organism linked to disrupted acid regulation. What we see clinically is that when biofilm is present, conventional breath tests can miss strains — so we look carefully here.
- Parasitology — screening for intestinal parasites that inflame the gut lining and impair digestion.
- Digestive markers — pancreatic elastase (a read on your enzyme output), fat and fiber digestion, and short-chain fatty acid patterns.
- Inflammation markers — calprotectin and similar markers that tell us whether the gut lining is actively irritated.
- Immune markers — secretory IgA, which reflects the state of your gut immune defenses.
How Digestion and Reflux Are Connected
Most people think of heartburn as too much acid. In a lot of the cases we see, the trouble is weak digestion. When stomach acid or enzymes are low, food sits and ferments. Fermentation creates gas. Gas creates pressure. Pressure pushes contents back up through the lower esophageal sphincter — and you feel heartburn.
The same chain runs through bacterial overgrowth: microbes fermenting your meals in the wrong place create the same upward pressure. Stool testing is how we find these patterns instead of assuming them.

- Heartburn that flares after meals, especially carbohydrate-heavy ones
- Bloating, gas, or a "food baby" sensation alongside reflux
- Unexplained bowel changes — constipation, diarrhea, or both
- A suspected or treated H. pylori infection that keeps coming back
- GERD symptoms that persist despite normal endoscopy results
- Long-term antacid or PPI use with unresolved symptoms
Conventional Stool Testing vs. Our Functional Panels
| Topic | Conventional stool testing | Our comprehensive stool analysis |
|---|---|---|
| What it screens | Pathogens (infection) | Pathogens plus microbiome balance and digestion |
| H. pylori | Breath test or basic stool test | Stool antigen and functional panels, looked at closely |
| Digestion markers | Rarely included | Elastase, fat and fiber digestion, SCFA patterns |
| Inflammation | Only when disease is suspected | Calprotectin and immune markers on every panel |
| Use for reflux | Not designed for it | Built to explain functional reflux drivers |
How Collection Works
- We mail you an at-home collection kit with clear instructions.
- You collect the sample at home — no clinic visit required — and mail it to the lab in the prepaid packaging.
- Results are typically back within 7 business days.
- We schedule a dedicated results review where we go through every marker with you, page by page.
No colonoscopy prep, no sedation, no waiting room. Most patients tell us the hardest part was remembering to mail the kit.
What Happens After Your Results
Results without a plan are just paperwork. At your review appointment, we translate the markers into plain language: what is overgrown, what is missing, what is inflamed, and what that means for your reflux. Then we build your care plan around the findings. That may mean targeted changes to what you eat, support for the digestive function your markers show is low, and a stepwise approach to calming the inflammation we found. Where medication is involved, we build any step-down with your prescribing doctor — see PPI cessation support.
Stool testing is often paired with the 184 food sensitivity panel or adrenal stress testing, since gut trouble rarely travels alone. It is one core piece of our 3-Step Root Cause Heartburn Protocol, and it is available by mail anywhere in North Carolina through Wilmington Functional Medicine.
What does a comprehensive stool analysis test for?
It evaluates your gut microbiome balance, bacterial overgrowth, H. pylori, intestinal parasites, digestive markers like pancreatic elastase and fat absorption, inflammation markers such as calprotectin, and gut immune markers. Together these show whether your reflux is connected to infection, weak digestion, microbial imbalance, or inflammation in the gut lining.
Do I need to stop my medication before stool testing?
Some supplements and medications can influence results, but that is a case-by-case question. We review everything you take before sending the kit and give you written instructions on what to pause and for how long. Never adjust a prescription on your own — we coordinate with your prescribing doctor whenever a change matters.
How long do stool test results take?
Results are typically available within 7 business days after the lab receives your sample. We then schedule a dedicated results review where we walk through every marker with you and outline the next steps. You will not get a report dropped in a portal with no explanation.
Can stool testing find H. pylori?
Yes. We use stool antigen and functional panels to look for H. pylori directly. What we see clinically is that conventional breath tests can miss strains when biofilm is present, which is one reason people test negative yet keep classic symptoms. If H. pylori shows up, your plan accounts for it directly.
